Support Coordination:

We can provide Support Connection, Coordination of Supports and/or Specialised Support Coordination. We endeavour to provide guidance to strengthen the participant’s capacity to connect to informal, mainstream and/or NDIS funded supports and services. We have specialised Nurses that can help determine if you are eligible for further assistance if the participant’s funds on their NDIS plan is almost depleted. If so, our Support Coordinators can prepare the required documentation to request a review on their NDIS plan.
